Registration Form Uploader

Use this uploader (drop & drop or Select files) to upload ALL your registration forms. Please used the following RULES when uploading your forms:

1. Name your file as follows: <Player Last Name, Player First Name - Form Name as above> (Example: “Kanahele, Kainalu - Poʻohala Waiver”)

2. Use PDF files whenever possible. JPEG for pictures are ok.

3. One file per each form. Otherwise, please add the page number to the end of the file name if youʻre uploading multiple files for one form.
